Prepare the request
Include the full page URL, the exact statement, the proposed correction, and a direct link to a reliable source. State whether the issue concerns a directory record, editorial wording, a broken source, or a technical problem.
Protect private information
Do not include medical records, claim numbers, government identifiers, confidential communications, or other sensitive personal information. The correction channel is public.
Submit and document
Open a public repository issue. The record makes the request, supporting source, decision, and resulting change auditable. Unsupported requests may be declined, and a firm is not removed solely because its website is temporarily unreachable.
